Hyperkalemia in fatal MDMA (‘ecstasy’) toxicity

Abstract

3,4-Methylenedioxymethamphetamine (MDMA, ‘ecstasy’) is an amphetamine synthetic analog widely used as an recreational drug. Acute and severe toxic effects following MDMA ingestion include hyperthermia, arrhythmias, rhabdomyolisis, disseminated intravascular coagulation, hepatotoxicity and even death [1], [2], [3]. Recently, we treated a patient in whom hyperkalemia, in the absence of renal failure, aggravated the expected toxic complications of MDMA, becoming the immediate cause of his death.

    Most of the lethal events involve hyperthermia as the cause of death (9). We found only one case of Ecstasy intoxication with hyperkalemia as the immediate cause of death after pure MDMA intoxication (10). One other patient was recently treated with dialysis but died 4 days later of multiple organ failure (11).
