Abstract

The contractile response to natural tachykinins and selective peptide agonists for tachykinin receptors was studied in strips of circular smooth muscle of human lower esophageal sphincter in vitro. The effects of phosphoramidon, which inhibits neutral endopeptidase (EC.3.4.24.11) and of the non-peptide compounds, SR 48968 and CP-96,345, which selectively block NK1 and NK2 receptors, respectively, were also investigated. Substance P, neurokinin A and neurokinin B produced a concentration-dependent contractile response. The rank order of potency was neurokinin A > neurokin B > substance P. Phosphoramidon (1 μM) potentiated the response to substance P without changing the order of potency of natural tachykinins. The NK2-selective agonist, ([βAla8]neurokinin A-(4–10)), produced a concentration-dependent contraction. The NK1 ([Sar9, Met(O2)11]substance P, 1 μM) and NK3 ([MePhe7]neurokinin B, 1 μM) selective agonists, however, did not exert any contractile effect. The selective NK2 antagonist, SR 48968, potently inhibited in a concentration-dependent (10 nM-1 μM) manner the response to neurokinin A, without affecting the response to carbachol. The selective NK1 antagonist, CP-96,345 (1 μM), did not affect the response to neurokinin A. These results indicate that tachykinins contract muscle of human lower esophageal sphincter, and that this effect is mediated by NK2 receptor stimulation. Moreover, a phosphoramidon-sensitive mechanism plays a role in the regulation of the response to substance P.

It has been particularly useful in preventing chemotherapy-, motion-, anaesthesia- or vanilloid TRPV1-induced emesis (Liu et al., 2015; Navari and Schwartzberg, 2018). However, the use of NK1 antagonists had been correlated with the appearance of diarrhoea (Bubalo et al., 2018; Yuan et al., 2016), and no literature had evaluated the (Huber et al., 1993) stomach and colon at the same time in an emetic animal model to explain this side effects. It is important to note that NK receptors are expressed in smooth muscle, ICCs and neurons in the GI (Drokhlyansky et al., 2020; Lee et al., 2015; Lee et al., 2017; Liu et al., 2015).

There are several studies that report that tachykinins mediate contraction of in vitro preparations of circular smooth muscle from the human lower esophageal body and that NK2 is the more important receptor. Thus the contractile response to natural tachykinins or selective receptor agonizts resulted from activation of NK2, but not NK1, receptors (Huber et al., 1993b), and NK2 receptor blockade inhibited the largest proportion of tachykinin-evoked responses (Kovac et al., 2006). The transcript level of the TACR2β isoform was much less than that of the α isoform, but it was consistently more than that of TACR1 or TACR3.

The mechanism of NMDA receptor activation on modulation of NANC contraction in LES is not well understood. It is known that in the NANC contraction processes, the stimulation of NANC neurons induce the release of contractile mediators such as substance P, neurokinin A and B (Huber et al., 1993; Smid and Blackshaw, 2000). Recent report by Ma et al. (2008) has shown that in renal pelvis and during renorenal reflex, the release of substance P is being modulated by d-serine.

Particularly in circular muscles, NKA at 10 μM induced more potent tonic contractions, which was approximately 2.4-times higher than that induced by BCh at 100 μM. The results are consistent with the previous reports demonstrating that NK-2R agonist induced vigorous contractions of muscle strip of the rat and mouse stomach and human lower esophageal sphincter and ileum [33–36]. Moreover, whereas L-732,138 had no effect on NKA-induced contraction, the NK-2R antagonist GR159897 significantly inhibited NKA-induced contractions in longitudinal muscles and GR159897 at 100 μM almost abolished contractile effect of NKA at 3 μM.
